Agreement
In consideration of my child's participation in Green Mountain Gymnastics Inc. (further referred to as GMG) events and activities, I hereby agree to be bound by each of the following terms and conditions:

  1. Eligibility: I agree to comply with the rules of GMG.
  2. Readiness to Participate: I will only participate in those GMG classes, events, competitions and activities for which I believe I am physically and psychologically prepared. Prior to participation, I will have practiced by exercise and will perform only those exercises, which I have accomplished to the degree of confidence necessary to assure I can perform them by myself and without injury.
  3. Medical Attention: I hereby give my consent to GMG and/or host Organization to provide, through a medical staff of choice, customary medical/athletic training attention, transportation and emergency medical services as warranted in the course of my participation in the event, except where such loss or damage is the result of the intentional or reckless conduct of one of the organizations or individuals identified above.
  4. Waiver and Release: I am fully aware of and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses associated with participation in gymnastics activities and events. I also release GMG of liability for anyone associated with this gymnast in the event of injury that may be incurred within GMG's premises.

I further agree that GMG and the sponsor of any GMG event, along with the employees, agents, officers and directors of these organizations, shall not be liable for any losses or damages occurring as a result of my/our child's participation in any GMG event or activity, except where such loss or damage is the result of intentional or reckless conduct on one of the individual (s) /organization (s) mentioned above.

    Rules and Policies for Daycare Programs
    2008/2009 Season

    Arrival of Class
      If you are coming to observe, please note that parking for GMG is located directly in front of our space or along the side of the building. Please do not park in front of the door or in front of the other business in the complex. Be careful when walking in the parking lot during the winter months, it sometimes gets a bit icy.

    Closings
      If GMG needs to cancel classes, we will arrange a make up date for your group. If the daycare cancels due to whatever reason, a makeup will not be offered. There are no refunds for missed classes.

    Clothing
      All children must dress appropriately. We recommend T-shirts and pants that do not have buckles, zippers or snaps on them. No socks or tights. No Jewelry except stud earrings. All girls with long hair should have it tied back away from their face.

    Drop Policy
      If you need to withdraw from the gymnastics program during the session, we do require that we receive a written 2-week notification. If notification is not given, no refund will be given. This is required because in order for GMG to provide the children in your daycare the opportunity to come, we must have a certain number of children enrolled. We staff based on the number of attendees and we transport free of charge to the daycare as long as we have 10 attending in each group.
      If you are interested in continuing your gymnastics program, we offer weekday and weekend classes. Please call us and we would be happy to look to see if any other class times work for your schedule.

    Emergency Numbers
      It is extremely important that you update us whenever there is a phone number change. This is especially important if there is an injury and we need to reach you. Please drop us a note or send us an email at Robin@GreenMountainGymnastics.com so we can make those changes to your emergency contact information.

    Illness / Missed classes
      If your child is not feeling well, we will not allow them to participate in class. Our instructors' job really requires them to work with the class on the gym floor and we do not have extra staff available to watch over kids that cannot participate. We do require you to be illness free for 24 hours before retuning to gymnastics class. If a child has an injury such as a broken arm etc, we cannot allow them to participate in our programs until we receive a note from the doctor giving your child permission. There are no refunds for missed classes.
      GMG enforces a No nit / No lice policy. If your child has lice, please notify us so that we can take measures to treat the gym so hopefully no other manifestations will take place.

    Lost and Found
      If anything is left behind after class, it will be put in the lost and found box. Every month this will be cleaned out and items will be disposed of. We will not be responsible for lost or stolen items. Please have them leave at home or daycare anything other then what they require to participate in class.

    Payment of fees
      Tuition for our Tiny Tumblers program is $160.00 for the 10-week session and $144.00 for the 9-week session. Please see the billing calendar for dates that payments are due. If you will be out of town or not attending class, please mail your checks so that your payment is received here on time. All checks should be made out to Green Mountain Gymnastics. There is a late fee of $5.00 per week per child for all payments received after the due dates. Please understand that we simply need to have all fees at the beginning of each session because we also have financial obligations that are due. Thankfully bounced checks are rare. However, if you do make a mistake and write a check that is rejected by your bank, we will charge back to you any charges that we incur and an additional $20.00 insufficient fund fee. We also ask that you come in and pay the balance due and those fees immediately, so as not to incur additional late fees.

    Programs
    We offer a number of programs for gymnasts of all ages. For more information on these programs, please visit our website @ www.GreenMountainGymnastics.com

    • Mom Pop and Tot - Preschool programs for children between the ages of 2 and 4. These classes are held on weekday mornings and last 40 minutes.
    • Girls and Boys Recreational Classes for ages 2 years and up
    • Junior High and High School non-competitive programs
    • Home school programs (call us to schedule time for your group)
    • Intermediate and Advancedclasses
    • USAG Competitive Teams, levels 4-10
    • Tiny Tumblers Open Gym for kids ages 6 months - 5 years
    • Kids Night Out (ages 6+)
    • Tumbling classes
    • Birthday Parties

    Substitutions
      If a child misses a class or decides they no longer want to participate, we do not allow someone else to take that child's place. Only the person who is enrolled at the time of registration is allowed to participate in programs. If you have someone new that joins your center and wants to join in our programs, we can add them at any point. We would pro-rate the first bill so that it follows our billing cycle. No one is allowed to participate in any class unless all paperwork is complete and turned in.

    Transportation
      If GMG is providing transportation for your center, you must keep in the child's file a signed note saying they give authorization for GMG to be picking them up.

    Viewing
      We encourage parents to come and watch at any time during their childs program. Only those that are enrolled in class are allowed onto the gym floor.

    Waiver Release Form
      No one will be allowed to participate in any program unless both parents sign the waiver release form, even in situations where the parents may not reside together. If anyone has a question on this, please call me and I will be happy to explain. For the safety of everyone, we remind you that only those enrolled in that particular class with a signed waiver form will be allowed on equipment.

    Parents/Day Care Providers
      Please review these rules in this book and be sure you understand them before signing our registration form. We ask your cooperation in helping your child understand the importance of safety within the gym. Remember that they learn best by example.
      It is the responsibility of the daycare to make sure all paperwork is filled out completely before any groups are sent into the gym for programs. If waiver forms are not properly filled out, we will not allow a child to participate in class.
      When entering the gym, please remove shoes and place them on the shoe racks. Check the lost and found box often. Please place tuition in the tuition envelope located at your daycare. We also ask the Daycare staff to bring in an extra outfit in case a child has an "accident".
